A form is a variant of tabular data presentation, for example, in MS Access it displays data from a table one record at a time. There are several ways to create a form, depending on the purpose. The main purpose is the ability to record and view information in the database, print data.

Instructions
Step 1
Start the MS Access program if you need to create a new form in the electronic database. Open your database and make sure you have at least one table in your database that will serve as the basis for work.
Step 2
Go to the "Forms" tab of the database window. Press the button "Create" and from the list select the command "Wizard" and the table, which will be the basis for further actions. Next, select the required fields from it, select the field and click on the arrow on the right. If you want to set all fields from the table, click on the double arrow. To construct a form with data from several tables, select another table from the list and add its fields in the same way.
Step 3
Go to the next wizard window by clicking the Next button. Select the option of placing data in the form: "One Column", "Tabular", "Aligned", "Striped". Click "Next". In the next window, select the appearance. In the next window, enter a new name and click the "Finish" button.
Step 4
Use the designer for work, for this, in the database window, go to the "Forms" tab and click the "Create" command. Select the desired table and the "Design" creation option. A new blank window will open. Go to the View menu and click the Field List command. Add table fields using this command. Place them in the window as you like by dragging the upper left corner with the mouse pointer in the form of a hand. To add control elements during design (buttons, images), click on the toolbar (the button with the image of tools). Save all changes.
Step 5
Create a form on a web page using a tag. To do this, be sure to use an end tag. It is also necessary to use the following attributes: accept-charset - defines the encoding in which data can be sent to the server, action - in this attribute set the program that will process the entered data, name - enter the name of the form, target - the window in which the data will be placed result.
