For the best driver uninstallation, use programs such as CCleaner or Your Unistaller. The second program is better, as it completely removes the software, clearing the registry of files related to the driver. It's easy to use: run the program, then find the driver in the list, click on it, then on the "uninstall" button (the trash can icon), then click "next" and just follow the program prompts until you completely remove the driver.

Right-click on the "my computer" icon, then click on "properties", then "hardware", "device manager", find your modem there in the drop-down list "modems", right-click on it, then "properties" and "driver".
