Step 1
On the first page of the form, enter the following data: the name of the legal entity or the surname, name, patronymic of an individual entrepreneur, private lawyer, notary or other person who opened a current account.
Step 2
Fill in the OGRN column.
Step 3
Enter the contact phone number in the form, and at the bottom of the page put the date and signature.
Step 4
On the second page, enter the following data: TIN, current account number, date of its opening, the name of the banking structure in which it is opened, as well as the BIC and the bank's correspondent account.
Step 5
Please write the correct full postal address and bank code.
Step 6
Sign at the end of the page.
